Please read ALL information on this page before booking an appointment.

WE ARE MOVING! Starting on February 24, 2025, appointments will be held on the Northeast side of campus in Modular 7. This is located between Lind Lecture Hall and the Interprofessional Education Building. You can purchase a day-use parking pass for $4 at the Information Booth on the West side of campus and park in the W4 or W5 parking lot, otherwise you may be ticketed. Our new location is handicap accessible.

Our office offers printed passport photo services for $10.00 for two images. You can get your photo printed at your appointment with us.

Passport Application Information

Please read all information thoroughly! If documents are missing or incomplete, you will be required to reschedule.

First Time Passport Application

If you've never had a U.S. passport, are 16 or under, or aren't allowed to renew an existing passport by mail, you will need to apply in person.

Step 1: Gather Necessary Documents

  • Application forms (unsigned)
  • Proof of identity document and a photocopy of the front and back
  • Proof of U.S. Citizenship document and a photocopy of the front and back

Step 2: Prepare Application Package

  • Complete your  on the State Department website.
  • Print your completed application. DO NOT SIGN YOUR APPLICATION. A Passport Agent must witness your signature.
  • Have a passport photo taken. Our office can take a passport photo for you at your appointment. Cost is $10. Do not wear glasses or camoflauge, and be prepared to move all hair out of your face.
  • Photocopy your proof of identity and U.S. Citizenship documents.
  • Calculate your fees. . You must come with TWO (2) payments PER PERSON.
    • Execution Fee: The Weber State Cashiers office accepts credit cards for the execution ($35) and photo ($10) fees. This will be paid online during your appointment. 
    • Passport Application Fee: State Department passport application fees are mailed with your application. This will include the passport application fee, expedited fee (if applicable), and 1-2 day shipping (if applicable). You can pay with a personal check, certified check, cashier's check, traveler's check, or money order, payable to "U.S. Department of State." Starter checks are not allowed. These checks generally start with a "0". If all you have are starter checks please obtain a cashier's check or money order. PAYMENT MUST BE EXACT, so writing a check for an adult passport book, plus expediting, plus 1-2 day shipping must show $211.36, not $211.00 or $212.00.

Step 4: Attend in person appointment and pay processing fees

  • Processing fees vary by application type. Please find an updated . Remember, you need TWO different payments-- one as a check or money order for the US Department of State for your application fees/expediting/1-2 day shipping, and the other as a credit card for the $35 execution fee and $10.00 photo.

Renewing Passport

  • Renewing Passports: If you answer 'Yes' to all 5 eligibility questions below you can mail your renewal form directly to the State Department.

Your passport:

  • Is in your possession to submit with your application.
  • Is undamaged other than normal "wear and tear."
  • Was issued when you were 16 or older.
  • Was issued within the last 15 years.
  • Was issued in your current name or you can document your name change. 

If you don't meet all the renewal requirements, you'll need to do a new, first-time passport application and make an in-person appointment.

 

 

Passports for Minors under 16

  • Both parents/guardians must authorize the issuance of a child's passport. The best way to give consent is for both parents/guardians to go with the child when applying.
  • Children need evidence of citizenship.
  • Parents/guardians need proof of parental relationship.
  • Children's passports are only valid for 5 years. When they expire they will need to make an in person appointment to apply for a new passport.

Processing Times:

4-6 weeks processing time and up to 2 weeks of mailing times. If you have an urgent need to travel please visit this State Department page: 

Citizenship Evidence Documents:

  • U.S. birth certificate 
  • Full validity, undamaged U.S. Passport
  • Consular Report of Birth Abroad or Certification of Birth
  • Certificate of Naturalization
  • Certificate of Citizenship 

Passport Photos:

  • The correct photo size is 2 x 2 inches (51 x 51 mm).
  • Eyeglasses are not allowed in Passport photos. If you cannot remove your glasses for medical reasons, you must submit a signed statement from your doctor with your Passport application. 
  • The U.S. State Department specifies that photos must have a neutral facial expression with both eyes open and mouth closed. 
  • Don't wear a white shirt, uniform, or camouflage patterns for your photo.
  • Take off your hat or head covering.
    • If you wear one for religious purposes, submit a signed statement that says it is religious attire worn daily in public.
    • If you wear one for medical purposes, submit a signed doctor's statement that says you wear it for medical purposes.
    • Your full face must be visible and your hat or head covering cannot cast shadows or cover part of your face.
