Hours: Monday–Friday: 8 a.m.–5 p.m.
The first exam will be administered at 8:30 a.m.
The last exam will be administered at 3 p.m. and must be completed by 5 p.m.
Students are highly encouraged to schedule their exams at least 48 business hours in advance to help ensure a smooth testing experience. This also allows sufficient time to arrange necessary accommodations and verify availability in the Accommodated Testing Center. By planning ahead, students can maximize their chances of accessing the testing environment that best suits their individual needs.
All exams must be completed by the time the office closes, unless other arrangements have been made in advance.
Accommodated Testing Center Procedures
- If you need to schedule an exam outside of our office hours, please contact our office at 801-626-6896.
- Disability Services (DS) Testing adheres to the same guidelines as other 91风流 testing centers. Food, drinks, hats, glasses, bags, etc. will not be permitted in the testing rooms.
- Students must be authorized to receive testing accommodations. If you are unsure if you are authorized to receive testing accommodations, or are unsure about what the accommodations are, please make an appointment with your advisor.
Scheduling a Quiz or Test with Disability Services (DS)
Schedule a test by contacting the DS front desk staff in person or over the phone.

Before contacting DS to schedule your test, be prepared with the following information:
- Name and "W" Number
- Course Information: instructor name, course name and number, days and times course is taught
- Day and Time you wish to take your test
- DS stops giving out tests 2 hours before our posted closing time. Be sure to schedule your time wisely. Disability Services offices have different testing days and times to accommodate student schedules.
- Type of Test: Paper, Scantron, or Canvas
- Where DS can pick up or drop off the test: specific testing center, department office, Chi Tester
- Any additional accommodations or needs you may have regarding your test
If you cannot make your testing appointment:
Please contact Disability Services as soon as possible to reschedule. Students who show up late to their testing appointment may be asked to reschedule for another day or time.
Test Administration Procedures
The following procedures must be followed by the student and employees to ensure the security and validity of all tests administered by this department. Any deviation from these procedures must be reported immediately to the Accommodated Testing Supervisor.
- All paper tests given by or proctored by this department must be picked up and returned by a badge-wearing employee or the individual instructor; under no circumstances are students permitted to pick up or return exams unless accompanied by an authorized employee.
- Unless specified in writing by the instructor (or approved as an accommodation), students are not permitted to have notes, books, calculators, or other materials in the testing room.
- Office personnel will periodically perform a sight check on students taking exams, and testing rooms may also be monitored by camera to ensure test security and fairness.
- Test Readers are to act only as "eyes" for the student and are not permitted to add or delete material; no information can be inferred through body language or voice inflection.
- Test Scribes will only write exactly what is dictated to them by the student, including punctuation, capitalization, and other grammatical details.
- Students are required to complete the exam in one sitting unless arrangements have been made before the exam begins.
- No student will be allowed study time to prepare for a portion of an exam that the student has already seen or heard unless a specific accommodation has been arranged.
- Unless an instructor provides written approval to keep specific items (such as a study guide), all materials—including scratch paper, notecards, and any notes taken during the exam—must be submitted to staff before exiting. Students are strictly prohibited from copying, recording, or electronically transmitting any exam content for use after the test is completed.
- Electronic devices, including cell phones, smartphones, smartwatches, tablets, and eReaders, are not permitted in the testing rooms without a formal accommodation.
- Students using personal medical devices should notify their disability advisor and testing personnel in advance if the device will be needed during an exam.
- Restroom breaks are only permitted with an authorized accommodation or with prior permission by the instructor.
- All paper tests administered by this department will be accompanied by a test tracking form to ensure all necessary information and timestamps are recorded.
